Health Information Forms

You can also fill out the forms in advance and bring them with you to your first visit by visiting our Forms section of our website and print them ahead of time.
Once your forms have been completed, you will have a consultation to discuss your health-related problems, concerns, as well as potential treatment options.
History and Examination
After your consultation, we will ask you various questions related to your condition. Next, a number of specialized (non-invasive) computerized spinal scan tests will help determine which soft tissues such as nerves and muscles are affected and to what extent. Also performed are several orthopedic and neurological tests along with a range of motion and postural exam. example of the computerized spinal scan
